Diploma Issue & Replacement (Duplicate Diploma Request)
Diploma Issue
Diplomas are mailed by the Wednesday after your degree was conferred. If, after 15 business days from the date of the issuing of your diploma, your diploma has not arrived at its intended destination, notify the Graduation Office at (216) 687-3700 or at graduation@csuohio.edu. Notifications of non-receipt are accepted up to 60 days after your diploma has been issued. After 60 days, you must order a . All notifications of non-receipt will be investigated. The Graduation Office reserves the right to refuse the complimentary re-issuing of a diploma.
Diplomas can also be re-issued if they are damaged or bent when delivered via mail. If this is the case, Diplomas that have been mailed and are folded, torn, or destroyed, can be re-issued for free. Student must produce the destructed document (deliver in-person to BH 116 Allin1 Enrollment Services office) or take a photo and email to the Registrar at graduation@csuohio.edu to have one re-issued. If you discard the diploma before taking a picture-- you must contact graduation@csuohio.edu within the time aforementioned to have it re-issued.
Diploma Replacement
If you need a replacement diploma or an additional copy (duplicate) of your diploma, please complete and submit the form with a check or money order to:

Express Mail
If you wish to have your diploma sent via Express Mail-- you would first register with and then email the shipping label to graduation@csuohio.edu with your form. You would also pay the fee online via . Please note: Based on the time you place and submit your order, the submission of the label does not guarantee the duplicate diploma will be mailed the same day.
Notarization & Digital Requests
If you need a replacement copy of your diploma notarized, please indicate your request on the Duplicate Diploma Request Form.
We do not provide digital copies or pdfs of diplomas.
